Hi, I am trying to make a survey in the same format as the one in FYWO and was wondering if anyone knows how to do it? I already looked for a survey website and, obviously, couldn't find what I needed. Please help me :o)

Thank you for that link, it did help some, however, I was hoping to make my own survey with specific questions and needed advice on that. If anyone has any info regarding that I would be very much glad to hear it. Thanks again :o)
